When should I use this form?

Use the above form to file a report about any specific incidents, concerns, or complaints involving USM staff, students, and/or community members. This can include:

• Title IX Incidents:

  Please use this form if you feel that you have witnessed, or been the recipient of, sexual or non-sexual: 1) discrimination, 2) harassment, 3) violence, 4) dating violence, 5) stalking, or 6) anything that you feel may be related to one of these options.
  
  

• Concerns about students:

  Faculty, staff, students, and others may refer a student who may be struggling with academic, personal, or emotional difficulties or who may be exhibiting concerning behavior.
  
  

• Handbook violations:

  Students, faculty, and staff may report incidents of violations of the Student Handbook Code of Conduct and/or Residence Life Handbook.
  
  

• Academic Dishonesty:

  Faculty to report incidents of student academic dishonesty, including supporting information, evidence, and relevant correspondence.
